Required Paperwork

Families enrolling a child in the Hillsboro, IL Pre-K program will need to provide the following documentation:

Health & Identification

  • Current physical examination

  • Up-to-date immunization records

  • Lead screening

  • Official birth certificate (issued by the courthouse)

Proof of Household Income
Provide one of the following:

  • WIC

  • SNAP

  • TANF

  • Parent’s medical card

  • W-2

  • Most recent tax return

  • Recent pay stubs
